Expression of Interest Form (Form 100) If you are interested in sports chaplaincy now or in the future fill out this expression of interest form. SCA Associate Member Form (Form 50) If you are involved with SCA or would like to be, eg. a committee member, ministry helper, volunteer, prayer team, etc... and not operational (a chaplain) this Associate Member form is for you. Police Checks All chaplains require current police clearances and checks with their applications. Please visit The Australian Government Families Website to find out how to get a check in your State.
